The Premarital Exam: Cringe or Clutch?

Did you know that in the 1930s, many states in the US passed laws requiring blood tests, including tests for STIs like syphilis? It began as a public health measure to reduce transmission of undiagnosed syphilis to newly sexually active people and reduce the risk of infants being born with congenital syphilis. In 1954, all but 8 states required applicants for marriage licenses to submit proof of premarital blood tests. It wasn’t until 2019 that this requirement was finally abolished across the country! Historically, “virginity testing” was an unethical and medically unnecessary type of exam designed to provide physical proof that someone had not yet had vaginal intercourse (cringe). There are many controversial beliefs and social issues related to that practice, and as such, some people may be hesitant or reluctant to consider any advice for couples or individuals to make a trip to the physician’s office prior to getting married. A trend to schedule a “premarital exam” has developed over the years in states with requirements for teachers to only teach abstinence, as well as those with large religious organizations or high numbers of people who believe sex should only happen within a marriage. Patients requesting these exams may have questions about having sex for the first time and often wish to discuss birth control. We believe all patients should openly discuss their sexual practices, medical history, and any concerns with their physicians, whether they are planning to get married or not and whether they have had previous partners or not. We are redefining the premarital exam to be an opportunity for all patients planning their weddings to establish care with a trusted partner in their overall health and wellness (clutch). Our wedding packages are specifically designed for patients planning to get married within the next 6 months to 1 year. Initially, we will gather pertinent health information about each partner and about the relationship. We will provide appropriate screening, helpful information, and support as you concentrate on all the wedding planning details. We will focus on the mental, physical, and sexual health of each individual and the couple, including evaluating and making recommendations for healthy communicating and maintaining your sense of self within your marriage. As always, as we get to know you and your partner, we will customize your care based on your individual needs.
